Subject:Request for InformationFrom:"Todd, Karmen (HHS/OCR)" <Karmen.Todd@HHS.GOV>Date:Fri, 17 Jul 2009 13:16:34 -0400To:Hillarie.........
Ms. Levy,
We continue to investigate your complaint and we are analyzing information that you and Kaiser have both provided. Below are a couple of follow-up question that I have for you to help facilitate our investigation:
1. Do you have records and know the specific dates when your daughter received blood transfusions during 2004 - 2005?
2. Do you know which of these transfusions included the donations made by you and other family members? If yes, do you have records and know the specific dates when your daughter received transfusions that included your donations?
(You previously provided documentation that you donated blood for your daughter on 1/22/04, 2/9/04, 6/16/04, 9/8/04, 10/4/04, and 2/4/05. We also have your written statements that William McCowan donated blood for your daughter on either January 20tth and 22nd or February 20th and 22nd.)
Thanks,
