Ms. Todd, Does that mean you have the information you requested? According to Kaiser, the information you requested has been sent to Mary Mata, Quality Improvement Director at 818-719-4536. My husband, Keith Levy, who at the time was president of the Temple in Simi Valley CA, remembers Eric Breiter donated blood to Robyn. He doesn't remember who else but we have the 2005 Temple membership directory where names can be matched. Also, the information one must fill out prior to donating blood, such as an address (a Simi Valley address indicates a Temple member, as this location is 20 miles from Simi Valley) and more import, this form indicates if this donation is a directed donation to Robyn.
