Out of 41 investigations reported in the State of California for all counties by the California Department of Public Health in the year 2012, 11 involved Kaiser Hospitals.  That equates to roughly 20% of all investigated com­plaints were of Kaiser Hospitals.  However, it appears that this information is based on the public lodging of com­plaints and nothing else.  If the public doesn't complain there is no true way to determine how serious any problems are with any hospital unless you want to blindly trust a paid published corporate PR statement.

California State hits Kaiser with 4 million dollar fine: The state of California Department of Managed Health Care said Kaiser made patients wait at least 14 days after an appointment before they could call to schedule another one, meaning a patient could not receive a treatment plan that was pre-scheduled for several individual appointments over a two- to three-month period among other things.

Dr. Andris Skuja, a Kaiser psychologist, offered strong words in reaction to the news of the $4 million fine and cease-and-desist order.

"This action confirms what every Kaiser clinician knows," Skuja said. "Kaiser doesn't take mental health care for its patients seriously." (as published in the Sacramento Bee --

Kaiser Permanente's Oregon-based unit has agreed to pay $1.8 million to settle charges of false billing brought by the federal Medicare program.

June 4, 2007 "Review of Vendor Rebates Paid to Hospitals-Kaiser Foundation Hospitals and Health Plans, Inc., Oakland, California (A-05-07-00051) The objective of the review was to determine whether Kaiser Foundation Hospitals and Health Plans, Inc. (the provider) reduced costs reported on its fiscal year 2003 Medicare cost report by the $35,438 vendor rebate it received.  The provider did not reduce costs reported on its fiscal year 2003 Medicare cost report by the $35,438 rebate, contrary to Fed­er­al regulations and CMS guidance. ...recommended that the provider (1) revise and resubmit its 2003 Medicare cost report, if not already settled, to properly reflect the $35,438 rebate as a credit reducing its health care costs and (2) consider performing a self-assessment of its internal controls to ensure that future vendor rebates are properly credited on its Medicare cost reports.  The provider agreed http://oig.hhs.gov/oas/reports/region5/50700051.htm
April 15, 2003 Kaiser Foundation Hospital –Sunset, Los Angeles, California, agreed to pay $20,000 to resolve its liability for CMPs under the patient dumping statute. The OIG alleged that Kaiser was a hospital that had specialized capabilities or facilities that refused to accept the transfer from another hospital of an 83-year-old patient needing Kaiser’s capabilities for coronary bypass surgery. The OIG alleged that Kaiser’s cardiac surgeon refused to accept the transfer saying that the patient was too unstable to transfer and that he was going to die anyway. The patient was transferred to another hospital where he underwent successful surgery and was discharged.

The Santa Rosa hospital is the third Kaiser facility to be cited by the state for using an unlicensed assistant during operations. Kaiser hospitals in Sacramento and in San Jose were also cited by the agency, which licenses and regulates the state's hospitals.

July 15, 2009 -- DENVER - Two units of Kaiser Permanente have paid a total of $367,000 for 2007 market violations.

The Colorado Division of Insurance, which announced the violations yesterday, conducted two market examinations covering the 2007 calendar year. Kaiser Foundation Health Plan of Colorado and Kaiser Permanente Insurance Co. received fines of $201,000 and $166,000, respectively. Payment was received on July 6.

The Division of Insurance, part of the Colorado Department of Regulatory Agencies, carries out market conduct examinations of insurance companies to ensure compliance with state insurance law and regulations related to rating, underwriting and claim practices.

The Kaiser reports identified a number of issues, many dealing with items in contracts and other forms. The reports also included issues with claims, such as failure, in some instances, to:

"Consumers count on health insurers to provide the services they have purchased," said Insurance Commissioner Marcy Morrison in the announcement. May 19, 2000 Kaiser received Medicare overpayments of $11,214 for 15 beneficiaries.  The 15 beneficiaries were part of a statistical sample of 100 beneficiaries reported as institutionalized between January 1, 1996 and December 31, 1998.  Based on these sample results Medicare determined that Kaiser was overpaid $49,963.00  for beneficiaries incorrectly listed as institutionalized.  Kaiser was advised to only refund the $11,214.00 although they were advised to investigate them­selves and if their figures agree with Medicare to return all the money.
